1. Application for appointment of a professor to the Scientific council chairperson stating the field of application.
2. Structured curriculum vitae with the following sections:
   • personal details,
   • education and professional development,
   • survey of work experience,
   • research fellowships abroad,
   • scholarly profile (survey of activities with quantitative data),
   • pedagogical background (review of activities with quantitative data),
   • membership in professional associations, editorial panels of academic and professional journals,
   • rewards,
   • competence in foreign languages,
   • other important information.
3. photocopied certificates verified by notary of second- (Mgr., Ing.) and third-level (PhD.) degrees.
4. photocopied certificate verified by notary in which a senior lecturer (docent) degree was awarded.
5. Educational activity and achievements in this area (§ 75 par. 5 act n. 131/2002 Z. Z.), structured as follows:
   1. study programme units (lecture, seminar, workshop, laboratory work, excursion, fieldwork, etc.) the candidate has been involved in over the past 5 years – the list signed by faculty dean,
   2. implementation of new study program units,
   3. designing study programmes, being guarantor, coursebook writing, producing teaching aids (over the past 5 years),
   4. supervising and critiquing theses (numerical survey),
   5. tutoring talented students (ŠVOČ and others – Students’ Research Activity),
   6. chairmanship and membership in commissions for dissertation thesis defences (numerical survey)
   7. chairmanship and membership (numerical survey) in viva voce and specialisation exam commissions (§ 46 act n. 131/2002 Z. z.),
   8. international cooperation in the area of education (in case of projects: principal investigator, deputy of principal investigator, or project co-researcher).
6. Confirmation of educational work experience issued by personnel office of the employing faculty.
7. Scholarly, research and field activities structured as follows:
   1. list of original scholarly publications, specialist works, coursebooks, teaching materials according to Smernice MŠ SR č. 8/2007-R dating 31 May 2007, which changes and supplements Smernica č. 13/2005-R of 31 August 2005 on bibliographic registration and categorisation of publishing activity and citation.
   2. project of grant agencies stating principal researcher or her/his deputy in the following structure:
      • project of international grant agencies (RP, COST, EUROCORE, A LA CARTE, NATO, NSF, JRC, other.),
      • project of national grant agencies (GAV, APVV, VEGA, KEGA, ŚP).
   3. Survey must be signed by the candidate and the dean of the candidate’s employing faculty.
8. List of proved quotations and citations from their scholarly and research works structured according the supplement to Smernica MŠ SR č. 8/2007-R, dating 31 Ma 2007, on bibliographic registration and categorisation of publishing activity and citation.
9. Projects and outcomes ordered by user institutions.
10. Achievements in academic tutoring and supervision activities:
    • completed-successful postgraduate (doctorate) students (name and surname, date of degree award, subject area),
    • postgraduate (doctorate) students after doctorate dissertation exam, that you supervise (name and surname, subject area),
    • postgraduate (doctorate) students before doctorate dissertation exam and other results in academic education proccess,
    • chairmanship and membership in docents habilitation committees.
